Once you are notified that you are covered you can book an appointment by using the book now button or contacting me directly. If you have booked your appointment prior to approval, no further action is required.

SELF-PAY:

 

If you are paying out of pocket, payment is due at the time of visit. If you have insurance, a receipt and superbill will be provided for reimbursement.
